Wedges and Revision Core Techniques

Deformity correction, lateral column lengthening, MTP fusion, and shear blocking techniques

Deformity Correction
Residual forefoot varus that is not corrected through other procedures can be addressed by a plantar flexing opening wedge osteotomy of the medial cuneiform, known as a Cotton Osteotomy. The ENHANCE® Instruments Set is used to insert a pre-shaped Cotton Wedge from MTF Biologics, which is made of dense cancellous bone with a two-side cortical rim to provide structural strength.

Lateral Column Lengthening
To treat patients with flatfoot deformity, a lateral column lengthening procedure can be used. The lateral column is made up of the calcaneus, the cuboid, and the fourth and fifth metatarsals. The ENHANCE® Instruments Set is used to insert a pre-shaped Evans Wedge from MTF Biologics, which is made of dense cancellous bone with a two-side cortical rim to provide structural strength.

First MTP Fusion with Metatarsal Head Bone Loss
Instability and bone loss of the first metatarsal phalangeal joint is a common pathology in foot and ankle surgery. To restore joint stability, a revision dowel can be implanted into the proximal phalanx and the first metatarsal. After that, the implant of choice can be placed overtop of the dowel.

Shear Blocking Technique for Difficult FusionsL Calcaneocuboid Joint
The calcaneocuboid joint is one that has a lot of sliding movement, which leaves it unstable as a result of injury or overuse. When proper stability is unable to be reached by non-surgical intervention, calcaneal revision cores can be implanted to stabilize the joint.
