LATEX INFORMATION

April 22, 2003
Re: Latex Content
To Whom It May Concern:

This letter is to inform you of Linvatec/Hall Surgical products and packaging that contain latex. At this time, the only products marketed by Linvatec Corporation, which contain any type of latex are as follows:

Irrigation Tubing Sets

Name of Device Catalog Number
Irrigation Fluid Pump Set (LPIS Pump)
60-5175-036
Quick Connect® Tubing Set (LPIS Pump) 60-5175-038
Aqua-pro Tubing Set C7039
Tube Set Arthroscopy (83000 Pump) 83100
Tube Set Non-Sterile (83000 Pump) 83100N
Tube Set Arthro Cassette, Use with 87000 Pump
NOTE: Lot number 0105101 and higher are latex free.
87100
Tube Set Arthro, Use with Small Joint Cannula System 83091
Small Joint Tubing and Cannula Set
NOTE: Lot number 58014 and higher are latex free.
9350

There are no alternative latex free tubing sets that are able to function with the LPIS Pump or Linvatec 83000 Arthroscopy System. However, our Apex® Universal Irrigation System utilizes latex free tubing sets, as well as our 87000 Pump System.

To the best of our knowledge, the only Linvatec product whose packaging contains latex is the Puddle-Vac® floor suction device, catalog no. 9321 (lot number 360063 and higher are latex free). This product contains a latex rubber band within the packaging.

If you have any questions, please contact the Linvatec Regulatory Affairs Department at (727) 392-6464.

Sincerely,
LINVATEC CORPORATION
